INSTRUCTIONS TO APPLICANTS:

Please complete all sections of the Application Form using BLOCK LETTERS. Applications for the Shares offered by this SSPS can only be accepted on this Application Form.

• A. From the eight alternatives, nominate which you apply for.
• B. Application monies must be for the full amount in Australian dollars. Cheques must be made payable to "Medical Monitors Limited – SSPS Account" and crossed 'Not Negotiable'. If your Application Form is not completed correctly, or if the cheque payment is for the wrong amount, it may still be treated as valid. The directors decision as to whether to treat your application as valid, and how to construe, amend or complete it, will be final.
• C. Applicants should carefully read the terms and conditions of the SSPS as set out in the attached letter;
• D. Cash payments will not be accepted, either via the mail or in person;
• E. The Directors may reject an acceptance of this offer or issue a lesser number of shares than that applied for, in its absolute discretion, if this form is not completed in accordance with this offer, is incomplete or if the exact amount payable is not provided with this form;
• F. The offer is not renounceable and as such may only be accepted by the shareholder printed on this application form.

This offer remains open until 5.00pm WST 5 December 2003.
