F1: The option vested as to 25% of the shares on the first anniversary of the date of grant and 1/36 of the remaining number of shares monthly thereafter until November 23, 2005 when the option became fully vested as to 100% of the shares to the extent it was not fully vested.

F2: The option is a "time-based" option that vested as to 25% of the shares on November 23, 2006 and as to 1/36 of the shares each month thereafter until fully vested on November 23, 2009.

F3: The option is a "performance-based" option that vests based on the determination by the Issuer's board of directors or compensation committee as to the satisfaction of certain performance criteria for each fiscal year 2006 through 2010. In February 2009, the board of directors approved the immediate vesting of the 2006, 2007 and 2008 performance-based options that did not otherwise vest during 2006, 2007 and 2008. In addition, the performance criteria for 2009 were met, resulting in the vesting of 100% of the 2009 performance options.

F4: The option is a "superior" option, which will become a performance-based option upon the closing of the initial public offering of the Issuer that vests based on the determination by the Issuer's board of directors or compensation committee as to the satisfaction of certain performance criteria for fiscal years 2010 and 2011.

F5: The option vests as to 25% of the shares on February 4, 2011 and 1/36 of the remaining number of shares monthly thereafter.
