I was holding 2 shares of Motor Industries Company Limited of face value of Rs.

100/- each in physical form.

I have acquired these shares in the year 1989 and got the shares transferred in my name. The above shares are misplaced / lost at my end; however I am in possession of covering letter of transfer of shares in my name dated 14.04.1989. A copy of the same is enclosed herewith for your kind perusal.

It is possible that my wife Bula Devi Binani might be the second holder in the aforesaid folio. She has expired recently in May 2021 due to COVID, a copy of her death certificate is enclosed herewith for your kind perusal.

Please do let me know the formalities to be complied with to get the Duplicate Share Certificates, and also the unclaimed dividend of these shares.

As discussed with you over mail, please find attached self certified copies of my PAN Card and Aadhaar Card duly notarised for your records.
